What services do you offer?

We offer personal training and group classes. For more information on our services click here.

Do you offer an open gym?


No, we do not offer an open gym. FORM is only available for personal training or group training.


If you are looking for an open gym there are other great options in Yangon.

Do you have lockers, changing rooms and showers?


Yes, we have a full bathroom, shower room, and locker amenities. Towels, body wash, shampoo, and conditioner are all included, free of charge.

How can I get a personal training Coach?


To start with a personal training Coach we need to understand your goal and training preferences. Please provide the following information to


  1. What goal do you want to achieve?

  2. Why is this goal important to you?

  3. Do you have a preference of working with a male or female Coach?

  4. What days/times would you prefer to train each week?

How can I get started with group training?

To join one of our classes and to guarantee your spot, you should book ahead of time. To book, you will need to purchase one of our packages and then book your session.

Can I turn up for a group class without booking online?


Yes of course you can, however, we cannot guarantee that you will get a spot in the class. 


We strongly suggest booking online beforehand to secure your spot and avoid disappointment.

What class is best for beginners?


All of our classes are programmed to accommodate all fitness levels. Our Coaches are there for your benefit and will help modify any exercises to suit you.


Obviously in the heat of the moment when the music is pumping and the weights are moving, it is very easy to push too hard too quickly (we’ve all done this!). We suggest that you go at your own pace throughout the session and take longer breaks if you need to. Remember, you aren’t competing with anyone but yourself, it is you vs you.


If you are new to FORM we suggest arriving 15 minutes before your class starts so you can chat to your Coach, let them know you are new and get the low down of the session before it begins.

Can I freeze or extend my package?


We offer freezes on our packages for a minimum of 7 days and a maximum of 30 days for: Injury, sickness or leave from Yangon only.  We must be told of the freeze before it happens by email to or providing our front desk with written notice. Please kindly note that we cannot freeze packages after the requested dates under any circumstances. We can only freeze packages ahead of time.


Once the date(s) of a freeze has been agreed, the freeze cannot be extended. In the case that you wish to extend the time the package is frozen, it will need to be done as an additional freeze of 7 days or more, and will be counted as another freeze on your package (as long as your package has another applicable freeze).


The freezes permitted on each package are as follows;


  • 1 month / starter packs / 1 session packages: Cannot be frozen

  • 3 months / 12 weeks / 10 session package: 1 freeze of 7-30 days

  • 6 months / 24 weeks / 25 session package: 2 freezes of 7-30 days

  • 12 months / 50 sessions: 3 freezes of 7-30 days


In the case of injury or sickness, please provide a medical certificate and we will freeze your account without it counting towards your permitted freezes. Please provide the medical certificate within 7 days of your sickness. Freezes for sickness without a medical certificate will count towards one of your permitted freezes.

What is FORM’s Refund Policy?


We do not offer refunds on our packages, all purchases are final.


In the case of relocation or long-term injury, we can transfer your package to someone else at the discretion of management. The package must be within the expiry period, the remaining sessions cannot be extended and must be used by the expiry date.
